Do I Qualify? 

Although cannabis can be a considered an alternative for many ailments, New Mexico state regulations and laws outline a list of approved medical conditions for medical providers to recommended cannabis treatment. Unfortunately, not all medical conditions would meet the qualifications for a   cannabis. New Patients must have a current medical assessment by their treating medical provider,  confirm their qualifying condition is chronic and debilitating. Canna Connection does offer full PTSD evaluations for those who may qualify under this medical condition, an additional assessment is required for this service if this is your interested service.

What Do I need to qualify?
(Must have all three)

Valid NM Drivers License or NM State Issued ID
& Email Address

NM Medical Program is open to NM residents only. Each qualified patient must present a valid NM drivers license OR NM State DMV issued ID. NO expectations. VA cards, Tribal Ids or Out of state IDs do not meet the application requirements. If you are a card holder for another state with a valid ID card from your home state, you may qualify under the Reciprocal Card Program of NM . 

All patients must also have a valid email address. NMDOH now requires all applications and card be issued and submitted using their online patient portal. In order to submit an application or receive your card, each patient must register their email. Only person is allowed per individual email. 

Qualifying Medical Condition

All new patients must have medical records documenting of the listed medical conditions. These records will need to be submitted by the patient or medical provider treating your condition. Canna Connection can request these records.

Medical Records must be reviewed and approved 

Medical records must be reviewed and approved for an appointment BEFORE an appointment can be arranged. The review of a patient's medical records are included in the new patient process. This step ensures that all patients meet the criteria of receiving a recommendation. This is not required to those who are new patients and receiving a full PTSD evaluation. You will have a two part process of your appointment to verify your diagnosis.
