Important Note: If you are new patient, without a valid or expired card a full evaluation or medical records are required for an appointment. If you complete a release of authorization, PTSD questionnaire or Anxiety questionnaire this will begin your application process.

 

No Medical Records? IF your medical condition is PTSD or Anxiety we do offer full evaluations for the above conditions only. Complete the questionnaire for your condition and you will be contacted for scheduling.

I Have medical records in the last year? If you have a medical person who is treating you for a qualifying condition we will need to request those records. You may submit your records by fax, mail or drop in during office hours.

Only those cards on the bottom expiration date need to renew. NEW REGULATIONS effective June 23, 2023

Only submit the appointment request if you are renewing or new and have your records. If you need a full evaluation or need us to request your appointments complete one of the forms that fits your needs, such as the medical release,  PTSD or Anxiety questionnaire.

All information is private and protected, any disclosure of your health information will only be authorized to the patient and or any individuals the patient approves.  IMPORTANT- If you submit this form on Thursdays after 2pm, your request will be reviewed the following Monday. Holidays and weather delays might also delay your request.

We do encourage patients to submit an online request or call for information on scheduling. We can accommodate walk ins, however to ensure each patient receives our undivided attention, we prefer arranging an appointment. It is important to note, new patients do require medical records as a requirement. We do request submitting or requesting us to obtain your records, prior to an appointment. This ensures we have the proper paperwork for your appointment. Renewals do not need documentation.
